Target Population
The target population is very low-income adults with mental illness and who are capable of living in independent community housing, with appropriate support services. Preference will be given to persons who have a history of psychiatric hospitalization and/or are at-risk for hospitalization; persons, referred from Division of Mental Health Services (DMHS) licensed residential programs, who are ready to move to independent housing; and consumers who are homeless and/or at risk for homelessness.
Exclusionary criteria:
- Documented medical condition related to organic brain syndrome.
- Physical conditions that prevent independent ambulating.
- Physical conditions that require professional medical nursing care and monitoring that would preclude a person from living in an independent setting.
- Persons with a history of violent behavior.
- Persons with a criminal history (felony).
